Put on your reading glasses if you normally wear them. |

Hold the grid about 15 inches away in good light. |

Cover one eye and look straight at the dot in the centre. |

Keep looking at the dot and notice the rest of the grid without moving your gaze. Then swap eyes. |
If any of the lines look wavy, blurred, faded or missing, mention it when the clinic calls. It is useful for Dr. Stein to know before you come in. The grid is not a diagnosis. It is a prompt, and the assessment is where you get a proper answer.
